1. About YouthLab

YouthLab (“YouthLab,” “we,” “us,” or “our”) provides provider-guided longevity, wellness, and health optimization services primarily through telehealth.

Our services may include consultations, personalized wellness pathways, health coaching, ongoing monitoring, and access to programs or products when determined appropriate by a licensed provider.

Our website provides information about our offerings and allows eligible individuals to request consultations and related services.

2. Medical Disclaimer

The general content available on our website is provided for informational and educational purposes only. It is not intended to replace professional medical advice, diagnosis, or treatment.

Clinical services offered through YouthLab are provided or supervised by licensed healthcare professionals. No provider-patient relationship is created simply by visiting the website, submitting a form, joining a waitlist, or requesting information.

A provider-patient relationship may only be established after completion of the required intake process, consent forms, identity and location verification, and consultation with an appropriately licensed healthcare provider.

3. Eligibility

Submitting a booking request does not guarantee that an appointment or service will be provided. We reserve the right to decline, cancel, or reschedule appointments based on provider availability, eligibility, safety, or clinical considerations.

b. Cancellations and No-Shows

Cancellation, rescheduling, deposit, and no-show requirements will be disclosed during booking or in your appointment confirmation.

Appointments cancelled without sufficient notice or missed without notification may result in a cancellation or no-show fee.

c. Appointment Preparation

You may be required to complete intake forms, provide identification, disclose your current location, submit medical records, or complete laboratory testing before your consultation.

Late arrivals may result in a shortened appointment, cancellation, or rescheduling.

5. Telehealth Services

Some YouthLab services are provided through video calls, telephone calls, electronic forms, secure messaging, or other telehealth technologies.

By participating in telehealth services, you understand that:

  • Telehealth may have limitations compared with an in-person examination.
  • Technical issues may interrupt or delay a consultation.
  • A provider may require additional testing or an in-person evaluation.
  • You are responsible for using a private location and secure internet connection.
  • You must not record consultations without prior permission.

A separate telehealth consent form may be required before services are provided.

6. Payments and Fees

Payment may be required before a consultation, laboratory service, assessment, membership, product, or other service is provided.

By providing payment information, you authorize YouthLab and its payment processors to charge the applicable fees.

Prices may vary depending on your selected program, provider recommendations, laboratory requirements, and other services. Prices may change for future services without prior notice.

You are responsible for any fees charged by your bank, credit card provider, financing provider, or payment processor.

Refunds, where available, will be governed by the refund or cancellation policy disclosed at the time of purchase.

7. Insurance

Unless expressly stated otherwise, YouthLab services may not be covered by health insurance, Medicare, or Medicaid.

You are responsible for confirming whether your insurance provider offers reimbursement for consultations, laboratory testing, prescriptions, or other services.

YouthLab does not guarantee insurance coverage or reimbursement. You remain responsible for all applicable fees regardless of whether your insurance provider reimburses you.

8. Health Information and Informed Consent

Before receiving services, you may be required to:

  • Complete a health and medical-history questionnaire.
  • Disclose current medications, supplements, allergies, and health conditions.
  • Provide laboratory results or other relevant records.
  • Review and sign informed-consent documents.
  • Confirm your identity and physical location.

You represent that all information you provide is truthful, current, and complete.

Incomplete or inaccurate information may affect the safety and effectiveness of your care. YouthLab is not responsible for outcomes caused by information that you intentionally fail to disclose or provide inaccurately.

9. Laboratory Testing and Home Phlebotomy

YouthLab may coordinate laboratory testing, home blood collection, specimen collection, or related services through independent laboratories and licensed service providers.

You are responsible for following all preparation, collection, storage, and shipping instructions.

Testing or processing may be delayed due to insufficient samples, collection errors, shipping delays, laboratory issues, weather, or circumstances outside our control.

Laboratory results should not be used for self-diagnosis or independent changes to your medications or treatment plan. Results should be reviewed with a qualified healthcare provider.

10. Prescriptions and Clinical Decisions

All clinical decisions are made by licensed healthcare providers using their independent professional judgment.

Completing an assessment or consultation does not guarantee that you will receive a prescription, medication, supplement, laboratory order, or particular treatment recommendation.

A provider may determine that:

  • A requested service is not clinically appropriate.
  • Additional testing or records are required.
  • An in-person evaluation is necessary.
  • You should seek care from another healthcare provider or specialist.

Any prescription medication must be used only as directed by the prescribing provider and dispensing pharmacy.

11. Compounded and Off-Label Medications

Certain medications recommended through YouthLab may be compounded by licensed pharmacies or prescribed for an off-label use when a licensed provider determines that doing so is clinically appropriate.

Compounded medications are not reviewed or approved by the FDA in the same manner as commercially manufactured, FDA-approved medications.

13. Third-Party Providers

YouthLab may use or coordinate services with independent laboratories, pharmacies, phlebotomists, technology providers, scheduling platforms, payment processors, delivery companies, or other third parties.

These providers may have their own terms, privacy policies, fees, processing times, shipping policies, and eligibility requirements.

YouthLab is not responsible for the actions, availability, delays, errors, or services of independent third-party providers where those matters are outside our reasonable control.

14. Intellectual Property
